Phillip Tolentino

Dr. Phillip ‘Phil’ Robert Tolentino, age 62, passed away on Saturday, July 6, 2024, at his home in Lakeland. Phil was born on May 22, 1962, to Drs. Felipe and Flora Tolentino in St. Louis, MO. He spent his early childhood in Manila, Philippines with his grandparents and grew up in the Boston area, going to Weston High School and Choate Rosemary Hall. Phil graduated from Colgate University and then followed in his family’s tradition of medicine at Chicago Medical School, where he graduated with a medical degree in 1995. At Chicago Medical School, he met his loving wife of 26 years, Karen Hulbert.


Phil loved to share his enthusiasm for sports: he played football, coached girls’ gymnastics, and spent time as a pitcher on a minor league baseball team. His friends and patients will remember his caring nature and warm smile. Phil’s family knew him as the person who could “fix anything”.


Phil is survived by his wife, Dr. Karen Hulbert. His sons, Alex and Nikolas. His father, Dr. Felipe Tolentino. His brother, Dr. Michael Tolentino. His sisters-in-law, Dr. Agnes Tolentino and Nara Detienne. His nephews, Ferdinand Percentie and Steven and Andrew Tolentino. His nieces, Amelia Percentie, Elizabeth Tolentino, and Natalie, Isabel and Reece Detienne.


Services for Phillip are being held at Heath Funeral Chapel & Crematory in Lakeland on Monday, July 29, 2024, at 11am.
